In consideration of my child being allowed to participate in any and all activities, programs, and services offered by Kicker's Sports Academy, I, on behalf of myself and my child, hereby acknowledge and agree to the following:

1. Legal Proceedings: I certify that my child has never been involved in any legal proceedings or convicted of a crime.

2. Custody and Guardianship: I acknowledge that there  are no custody or guardianship issues that Kicker's Sports Academy needs to be aware of. And if any issues arise KSA will be notified via email promptly.

3. Health: Kicker's Sports Academy will not be held liable for any injuries that may occur during their participation during Camp. I certify that my child(ren) doesn't have any allergies or medical conditions, allergies.

4. Insurance: I certify that my child has health insurance and that I have provided accurate and up-to-date information regarding the primary and secondary insurance providers. I further acknowledge that I have signed any waivers or releases related to my child's participation in sports activities.

5. Waiver and Release of Liability: I acknowledge and fully understand that there are risks and dangers associated with participation in sports activities, including but not limited to physical injury, death, or property damage. I accept and assume all such risks and, on behalf of myself and my child, release, waive, discharge, and covenant not to sue Kicker's Sports Academy, its owners, employees, volunteers, agents, and any other person or entity associated with Kicker's Sports Academy, from any and all liability, claims, demands, actions, or causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by my child or any property belonging to my child, while participating in any activities, programs, or services offered by Kicker's Sports Academy.

6. Indemnification: I agree to indemnify, defend, and hold harmless Kicker's Sports Academy, its owners, employees, volunteers, agents, and any other person or entity associated with Kicker's Sports Academy, from any and all liabilities, damages, costs, expenses, and attorney fees arising out of or related to any claims, demands, or actions brought by any third party arising out of my child's participation in any activities, programs, or services offered by Kicker's Sports Academy.

I have read this Waiver and Release of Liability, fully understand its terms and conditions, and acknowledge that I am signing it freely and voluntarily on behalf of myself and my child.
